Schedule: Day Shift | 40 hours per week
- Assist Health Providers in the planning, evaluating, implementing, supporting, and monitoring of patient care with the collaboration of providers.
- Review and track all incoming requests for referral and consultation, and coordinate all aspects of patient care with the interdisciplinary team.
- Arrange patient travel and send medical necessity letters as appropriate.
- Other duties or special projects as assigned.
- Travel to other locations/facilities may be required.
- Minimum of 2 year’s Case Management experience preferred, but not required.
- 1-2 years previous experience in Primary Care preferred.
- AHA-BLS, ACLS Certifications.
- Active AK RN License.
- 40-Hour Week Pay
- $2,100-$2,250/week
